Diagnosing ORA-3113 errors

Hi,

An application running on an NT workstation is experiencing ORA-3113 errors. I have never really been able to get a conclusive answer from Oracle on how to diagnose the problem causing these errors. If anyone has any suggestions they would be most appreciated.
