Use" Like Forms!frmName!FieldName & *" . Repeat this for each field using the "OR" criteria

 , etc.

Be sure you keep using the next lower row for each one

Like Forms!frmMN!field1 & "*"

                                  Like Forms!frmMN!field2 & "*"
