The easiest way is to call the 4.0 form in to 4.5, save it and generate it. Now for realities ... We found that the color pallates were not the same so we had to modify our forms to make them match what our users currently see. The most success we had was to open up the 4.0 form, open a new 4.5 form and copy objects from the "old" form to the new. Steve Cosner posted an article in response to "Something odd in my Forms 4.5" which listed the order in which he copied the objects. The important thing to note is that blocks should be last. Also be sure to delete a "null" or duplicate visual attribute that might be created before generating or you will donate more to the Gates Pension Fund (GPF).
