As far as doing the graphic stuff on a PC, I've used a package called ABC Flowchart for doing lots of ER/DF etc. diagrams. It's a very simple package, but is also streamlined, easy to learn, and fairly cheap ($300?). It can be a pain in the butt, however I haven't used/seen anything better. Micrographics (makers of ABC flowchart) also have a super fancy version called FlowchartWerks or some such thing. I haven't seen it, but I suspect they jammed a bunch of bells and whistles onto the original so they could triple the price.
