Forms 4.0.16 - LOV problem??? Strange

Greetings,

I have a very strange problem. I'm trying to get a List Of Values for a field (LASTNAME) from an outside table. But I'm having the hardest time.

Here is the situation:

	Oracle 		7.0.16.4.0
	Forms		4.0.12.4.1

	Block  		USER_INFO
	Text_Item 	LASTNAME
	LOV Name	LASTNAME_LOV

	LOV Name	LASTNAME_LOV 
	Record Group	LASTNAME_LOV

	Column Name 	LASTNAME 
	Title		LastName
	Display Width	40
	Return Item	USER_INFO.LASTNAME

	Record Group	LASTNAME_LOV
	Quenty		select fullname from user
	Data Type	Char
	Length		40

	Trigger	 Scope:	Block:USER_INFO / ITEM: LASTNAME
	Type:		Built-in - KEY-LISTVAL
	Text:		list_values;

	In debug Mode: 
	CTRL-L		FRM-42401: Performing program trigger KEY-LISTVAL
			on field USER_INFO.LASTNAME
	OK		FRM-40502: ORACLE error -- undable to read list of 
			values. 

I'm going crazy here... I've looked at everything else and I can't figure it out... does anyone see something I missed? or should have added?

Any help would be greatly appreciated.
