The kernel extensions were not loaded properly. Look in /etc/inittab and you should have three entries:

orapw:2:wait:/etc/loadext /etc/pw-syscall orakx:2:wait:/etc/loadext /etc/adspc
orakx2:2:wait:/etc/adspcinit

Check to make sure one of these is not misspelled (I believe one of the install scripts has an error in it - this should be documented in the release notes).
