O.Financials missing table help?

Hi there,

One of our forms is bombing because of a missing table:

        CS_ORGANIZATION_DOSSIER_NOTES Do any of you now know of a create script for this table? Who is supposed to own it? PO? MFG? SCOTT?

The form which needs this table is:

        $RA_TOP/forms/RAXCUDCI.inp

We are on mpl 9.4
