oracle server termination when using sqldba

Has anyone else encountered this intermittent problem? I'm running version 6.0.33.1.1 of sqldba with ORACLE RDBMS V6.0.33.1.1, transaction processing option. When I connect internal and type monitor process the program quits unexpectedly with a message like:

unexpected oracle server termination by signal 6

I then have to reset the terminal to proceed. A trace file is produced as below:
