HELP URGENT - Forms gen w/ Headstart crashes when opened

Hi,
I'm having this strange problem with forms (generated from Designer 2.1.2/Headstart 2.1) when running on the web. It seems that on the third call of a form in stacked mode (provided by headstart) it crashes giving an error indicating that a library can not be attached to the menu. After this message the form appear with the menu but all functionality of the menu is gone. This message is a bit misleading because it doesn't matter which form it is, if its opened on the third call it crashes with the same message everytime. But if the same form were to be opened on any call before the third it works.
ie.
A->B->C->D (D crashes)
A->D->C->B (B crashes) (D had failed last time but it work now because its not
the third)
Currently I am running Developer Server 2.1 on Solaris. I have tried the whole application in Motif and the same thing happens. It does however work on NT environment and that lead me to suspect its an environment thing. In headstart its states that the method in calling forms uses 'shared library', could this have an affect? Is this a resource thing on Solaris (both Developer Server and Form Developer on same box)?
Any helps, hints, suggestions?
