Migrating Forms 4.5 to Forms 6.0

Hi all,
does anyone know how to migrate existing forms currently in the database from Forms 4.5 version to Forms 6.0.
I don't have file system files (*fmb *plb and so on), just plain dump file. According to the existing situation FRM45__ tables are populated correctly in Oracle 8i database but apparently this tool can't find them.
