TK23W32.DLL blow-up using Forms 4.5.7.1.6

Howdy.

I've got this set of forms that I've developed in Forms 4.5.7.0.10
(Developer 2000 1.3).

I installed them on a Win 95 machine running Dev 2000 1.3.2 (Forms 4.5.7.1.6).
2 of the 3 forms work fine, but the 3rd is blowing up consistently at one point during a procedure being fired from the WHEN-NEW-FORM-INSTANCE

trigger.  It's GPFing in TK23W32.DLL.  Hep me!  Hep me!  It doesn't do this
on Forms 4.5.7.0.10 or Forms 4.5.6.5.5 or on the mac version of forms

(whatever that is at this point). It does GPF on NT 4.0 running Forms
4.5.7.1.6.

Yes, I've compiled the forms, libraries and menus for each platform.

Has anyone else run into this???? And better yet, has anyone been able to work around or fix it????
