Form's 4.5 rel 1.2 work with VBX.
Form's 4.5 rel 1.3 do not support VBX & OCX.

I read a few whitepapers about Forms&OCX, and in this articles writes:

Currently, Forms is not informed that Event has taken place in the OCX control.
... Developer/2000 rel.2.0 will include full support for OCX items that
will include the equivalent to the When-Custom-Item-Event trigger.
