Do you have more than one %oracle_home% in your path? Also check registry for multiple entries, and all the environment variables. See MOS ORA-12203 CONNECTING VIA SERVER MANAGER ON WINDOWS NT [ID 1041178.6]

Could be an effect of trying more than once, services screwed up. See MOS CREATE NEW DATABASE - CANNOT CONNECT: ORA-12203 AND NT ERROR 1058 [ID 1067887.6]
