I'm not going to mention the three big companys that have the practices I've seen, but they are as well known and large as these. Not to mention the numerous small companies with poor DBAs.

Look, when there are very qualified DBAs given the authority you lay out here, then, by all means, production problems are theirs. I don't want access to production. I do know that this is not usually the case, and many times I've been in production armed with a very deep application knowledge (which the DBAs have none of). Then, I'm on the horn with Oracle support, along with the dbas, getting it fixed.
