It is a patch that (in Oracle 10) is usually applied with 'opatch' and addresses a single bug.

Whenever you install a new release, all one-off patches are overwritten and gone.

Not all one-off patches are mutually compatible; opatch tries to keep track of that.

Basically you install a one-off patch if and only if you hit the bug.
