What are the contents of /var/opt/oracle/oraInst.loc? There should be a single path in there pointing to /opt/oracle/oraInventory.
Do you have an /etc/oraInst.loc? I do on 8.1.7 on RedHat7.1.
Also, if it says the orainstRoot.sh script isn't getting executed right, take a look at the script itself and walk through it step by step. Look at what it is supposed to do and make sure it is done.
