typically, there is a README type file inside the tar. Assuming you are on Unix, extract the tar file to a temp or staging directory. There should also be a unix.prd file in there which will contain the contents of the patch. Launch the installer program from $ORACLE_HOME/orainst, choose install from staging area, and point the installer to the staging/temp directory.
