I wonder from your description if it isn't more likely to crash at 16gb rather than 18. If this is testable then I'd test adding a 15.9 and 16g file. In any case what does the alert.log and/or trace say?. Similarly the contents of dmesg. Plus what Mark says about versions and setup.
