I have an OPS instance where both instances have the same name. Although both instances start and are accessable, when a failure of one instance occurs, the 2nd instance also fails. I was wondering if these 2 things could be related or if anyone has any experience with an OPS instance set-up in this manner.

If I remember correctly you can use the same SID on different nodes with OPS. However, when you upgrade to RAC, your instance names must be unique within the cluster.
