can we install forms 4.5 and 5.0 in the same machine? if that is the case which one has to be installed first? can anybody explain in detail

Yes, Forms 4.5 and 5.0 can coexist on the same machine, at least in NT. They install by default into different subdirectories, so it isn't even necessary to create separate Oracle homes.
