1-to-1's are quite easy to design in any "real" entity relationship diagram. Suppose you have a table for US_STATES and one for STATE_CAPITALS then there is a 1-to-1 relationship between them. In a relational model you can add the key of US_STATES to the STATE_CAPITALS and vice versa.
