For an Introduction to OODBs, I'd highly recommend:

Hmm... you didn't mention OODBs here. Not exactly what you are after, but I've done two books on OODB case studies:

The latter page doesn't have much right now, but the book will be published in October and has a couple of chapters on Object-Relational. Both books contain contributions from people with hands-on experience and contain lessons learned, architecture, design, etc. Have a look in a bookshop first.
