David Maier's Theory of Relational Databases is just that -- theory.

Anyone interested in database implementation should look at:

Database: Principles, Programming, and Performance, Second Edition by Patrick O'Neil, Elizabeth O'Neil
April 2000, 960 pages, Cloth, $74.95, ISBN 1-55860-438-3 http://www.mkp.com/books_catalog/catalog.asp?ISBN=1-55860-438-3 Note that the second edition has just come out, and the reviewers' comments at Amazon refer to the first edition.

For all the details, but unfortunately out of print, I recommend:

Database Design, Gio Wiederhold, McGraw-Hill, 1977. 007070130X

Or possibly the 2nd edition (March 1983) which I have not seen. I prefer this book to his more recent: File Organization for Database Design. 1987.
