Help! Toolbar Bitmap Buttons

How do I make a toolbar bitmap button on a Form?

I have set the TK21_ICON=path variable in the Oracle.ini, set the button properties: Iconic=True and Icon Name=my_icon, but the button still does not show any picture on the button.
