In Forms 4.5 use the File.Admin.Forms Doc to create a text file of
the code in the form.
I don't know much about Forms 3, but would this be enough to give you
the bare bones of the form?
The only thing you DONT get is the boilerplate labels on the canvas
(= Forms 3 pages).
