A few things come to mind:
- Are you sure you are using the same instance each time? It is possible (particularly if you run scripts) that executing the forms scripts may be pointing to a different ORACLE_SID.
- Have you gone into sqlplus and verified you can see the table? If so, then check the grants, roles, and synonyms.
- Have you verified you have access to the appropriate forms tables? The easiest way to verify this is to create a script that selects a count from table.
