SQL Forms error / Oracle error; help needed

We are running a straightforward forms application and some times when inserting data and later on trying to commit we get a strange error: UNABLE TO INSERT which gets back with an ORACLE error 00604 which basically says that there is a problem in a recursive SQL statement at level 1 and that dictionary tables cannot be accessed or something.

We have a hard time pinning down this problem. If anyone has had some similar experience, and has a pointer to what we should be checking, I would appreciate receciving a mail message.
