I was surprised to seel the thread on concatenated indexes. If there is an index on Col A + Col B, obviously, the index would not be used on a query on a value of Col B.
I see it this way : If there are different telephone books for different cities, to look up a person's number, you need to know the city he lives in. It doesn't matter if the city-name combo is unique - or whatever ! Where is the confusion ?
