I had a similar problem and logged an iTAR on the subject. I don't know what configuration you are running, but in my case, the problem was solved reinstalling JInitiator. The testform didn't work with JInitiator version 1.1.8.7, it did work with the older version 1.1.7.31.
My product specifications:
Windows 2000
Oracle8i 8.1.7
Oracle 9i AS v.1.0.2.2.2a
Internet Explorer 6.0.2600.0000
Netscape Communicator 4.79
JInitiator 1.1.7.31
