I hit a FRM-99999 problem deploying Forms Server 6i on a Unix box, and was pleasantly surprised to find that MetaLink had a thorough write-up on the various problems that raise the generic 99999 error. I don't have the document number, but try searching on FRM-99999 on MetaLink. As an aside, I have put both Forms 5 and Forms 6i on the web. Overall the 6i Forms Server seems much cleaner. It is faster, uses less memory per connection, and has fewer GUI-related bugs. If practical for you, you may find it worthwhile to upgrade.
