GPF while closing a Form (4.5)

I have a weird problem.
when am trying to close a form, windows is giving GPF.
i am able to open the form successfully.
No errors while compiling.
No errors while generating.
No errors while saving.
No errors while executing.
But, while closing the forms - Windows is Spitting A GENERAL PROTECTION FAULT. Is there anyone out there ? had similar proble.
I will appreciate your feedback.
Environment.
Forms 4.5
Windows 95
Against Oracle 734.
Form has attached libraries and some referenced objects. (I tried
deleting - it did not
help me)
