Multiple value select in Forms 5

I would like to gives users the ability to select multiple values in a Windows-like style, like the way CASE works when you select the columns to include in a view, or objects to implement in a schema:

One box would show the list of values, another box shows the selected values. Users could double click in the list to move items to the selected list, or ctrl-click or shift-click to get multiple values all at once.

Any suggestions for implementing this in Forms 5? Please reply to louiscoh_at_sprynet.com and/or lacohen_at_dreyers.com.
