VB5 (Win NT 4) crashes using Oracle Reports OCX

I am using the Oracle Reports OCX (R30sxa32.ocx version 3.0.3.9) in a VB5 program on Windows NT 4. Everything works fine until I attempt to exit the program, then it crashes. If I select cancel from the proffered dialog box, I am launched into Visual C++ and the following stack trace.
