UTL_FILE.INVALID_OPERATION exception, and I can't reproduce it!

Has anyone encountered a similar situation:
I have a loader scheduled to run overnight. Sometimes, for some files, the INVALID_OPERATION exception is raised when opening a LOG file is attempted.
Whenever I restart the loader for those same files, no error happens. I've also made sure that there is always enough space on the disk.
I'm running Oracle 7.3.4.3.0 on Windows NT.
