Forms 4.5 Menuing Statategies?

I've worked with Forms3.0 & 4.5 for awhile now, but I not sure how I might implement a menu of Forms for a user to choose from. For example, If a user has three forms available to them,

1) Add New Employee
2) Maintain Department Info
3) Maintain Job Codes

I would like to have the user start the application and be allowed to choose from these forms. Choosing a form would cause the form to start. One way I've thought of or maybe remember being used somewhere else would be to have a table lsting all the forms. Any suggestions/strategies on how other have implemented this would be welcomed.
