pl/sql vs commit form

We are still using forms 3.0 (I know it is not supported). We are running on a VAX platform on oracle 7.1. It seems to us that in some cases we do not get a complete commit when we do an insert with pl/sql. That is the information in the form gets commited but the information in the pl/sql does not. However if we put the values in a dummy block and do commit block we have no problems. The problem only occurs every couple of month in the former case. Has anyone every heard of anything like this?
