I hope you put the FORMS45_PATH in
HKEY_LOCAL_MACHINE->SOFTWARE->ORACLE->FORMS45_PATH. If you put it there, perhaps the value for FORMS45_PATH is to long. I had some trouble with the length.
I don't know the maximum length, but after shortening the value it worked again.
