Forms 4.5 OLE Object

I am trying to insert an OLE object in a Canvas. When I select to Insert Object, in the next window with the options to select the object, the three radio buttons with the possibilities (Create New, Create from File and Create Control) are showed disabled. I reinstalled my Forms 4.5 and it's showing the same.

How can I correct this problem ?
