Your problem may be related to a bug in Forms 4.5 which causes the above mentioned GPF. This is bug #308132. This occurs due to an error in the Oracle code for HINT.PLL. Any Forms which use this library are bound to get a GPF while invoking SHOWHINT.DLL (due to memory not being released).
