Two possibilities:

- You need to open and re-compile the menu with the new software.
- You need to add the directory the menu resides in into the NEW FORMS45_PATH. You can find this by running REGEDIT, double click on HKEY_LOCAL_MACHINE, SOFTWARE, ORACLE.
