Developing Oracle Forms Applications by Albert Lulushi published by Prentice Hall.
It's written for Windows, I'm a Unix weanie so I can't tell you how useful it is since about half of it doesn't apply to me.
It comes with a diskette and it's thick that much I know for sure.
