Adding : the "load_time" make me think that the table is being loaded with sql*loader (or something like that), and some bugs exists for loader donīt checking bad data if the loading is being done in directmode , like http://stackoverflow.com/questions/788136/sqlldr-corrupts-my-primary-key-after-the-first-commit , maybe this (or something like that) could be the case...
