For starters, Oracle Spatial is not middleware as it is part of the Oracle database. Even if you do not install Spatial, you still have some Spatial functionality with Oracle Locator, right in the database. There is no need to run any additional software.

There are differences between the two products....asking the differences between ArcSDE and Spatial is like asking the difference between Oracle and SQL Server (don't get us started down that road again!). The two products do similar work, but go about them differently. ArcSDE breaks the spatial data down into manageable components and then stores those components in multiple tables. Oracle Spatial uses object types instead. Oracle Spatial can take advantage of other Oracle features like Partitioning. But IMO, ArcSDE has better toolsets and other software packages to work with the spatial data, like ArcGIS, ArcToolbox.

This really is an open ended question and I'm not sure I really answered it for you. If you care to be more specific in your question, I will see if I can answer it. I have not worked extensively with Oracle Spatial, but I have worked with ArcSDE for many, many years. And I have looked into both products....
