Don't have any of this to test, but I would first ask if there is anything in an error log, particularly the http server log? I speculate it may be some silly protection problem on some file or directory. Certainly some fairly recent db versions have too-tight security in this vein. What is the db version, by the way? Have you restarted anything manually, and if so, how exactly? Are you using SSO and how is it configured?
