Upgrade to 9.2.0.7.0.
Dataguard 9.2.0.1.0 was full of bugs. I encountered such problems as yours with this version.

Try out the guidelines from http://tinyurl.com/mxsqz (3.2.1.4 Troubleshooting Switchover Operations)

And maybe just give up using the Dataguard 9i GUI: dgmgrl is much more reliable and lightweight.
