You can have one or all nodes on-line at the same time and unless performing maintenance all nodes should be started and in-use. If you don't have at least two ... you don't have RAC.

There are too many possible issues for me to diagnose anything from here ... Starting with the lack of version number and whether the cluster has ever actually been up.

I would suggest you open an iTAR.
