 The 4Gb boundary bug in early versions of 9.0 and 9.2 is particularly horrible. Can't offer any help on this point, though.

 Go to 9.2.0.4 immediately. The number of bugs fixed since 9.2.0.1 is enormous; in particular many critical security issues.
