Look at SQL Loader (an Oracle tool), not UTL_FILE (a PLSQL package). I fear you picked the wrong manual from the bookshelf... You can find more about SQL Loader here: http://download-west.oracle.com/docs/cd/A87860_01/doc/server.817/a76955/part 2.htm#435958
