I think that forms 4.5 use only SQLNET 2.3, not NET8. Install SQLNET 2.3 and copy your tnsnames.ora and sqlnet.ora form \ORAHOME\NET80\ADMIN to \ORAHOME\NETWORK\ADMIN.
