What are you using for your URL? Are you certain that it works? It should look something like:

http://www.yourhost/pub/plsql/test

where "pub" is your plsql agent name (I use the same name for the DAD associated with it).

Which is slightly different that in v.2x where you would have used something like:

http://www.yourhost/pub/owa/test

Also, I have noticed that on NT that this same error message pops sometimes intermittently for no apparent reason (v.3 only). Pushing reload or refresh usually takes care of the problem.
